IDATB’s Patient Portal is a website that was developed with the purpose of giving patients access to their personal health record, available on their computer, smartphone, or tablet. It drives patient engagement and self-service for increased patient satisfaction and practice efficiency. While this portal is not meant to be utilized as a substitute for our staff, we hope that you will find that it is a helpful tool for accessing your health information. Here, you can access your diagnostic test results, active and inactive medical problems, medication lists and medication allergies. You can also request medication prescription refills, send messages to providers and staff, and download forms. You are also able to modify your personal information so that our practice has up to date contact information for you. In order to register in the Patient Portal, you must receive an invite via email from our practice. These invites are usually sent to your email after your first visit, however if you do not receive the invite or cannot find the original email, please contact the office at 813.251.8444 and we can resend the invite to the email on file.

Please follow these instructions for registering/signing- in to the Patient Portal:

*Patient Portal upgraded on 8/14/17. Patients previously registered on old portal must follow instructions below.

  1. After you receive an email invite to register for the patient portal, go to the link listed in the registration invite email sent to you. If you have not received an email invite to the new patient portal, please call the office to request one sent.
  2. Complete the registration steps from the link, with the necessary information. You must enter the same personal information listed on your patient profile that we have in office. You will then be required to create a username and password, including security questions.
  3. Once you have completed the form in its entirety, and accepted Registration Disclaimer, you will have access to your personal health record at https://myhealthrecord.com, using the username and password you created.

Patient Portal FAQs

What are the services that the Patient Portal provides?

Currently, patients are able to view their diagnostic test results, health issues, medication lists, medication allergies and their most recent billing statements. Patients are also able to request medication prescription refills, personal information changes, as well as, message providers and staff. These new and exciting features will continue to evolve and in the future, we hope to offer additional options to serve the needs of our patient population.

Can I call your office if I have forgotten my password?

IDATB does not have access to patients’ passwords. Please utilize the “I forgot my password” feature on myhealthrecord.com

I submitted a clinical question yesterday but I have not yet received a response. When will your office get back to me?

The patient portal is not intended to act as a substitute for speaking with our staff. Please contact the office at 813.251.8444 if you have an issue that needs to be addressed immediately

Can I change my contact information within the Patient Portal?

Yes, once you request a change, that request will be forwarded to a staff member and we will be sure to make that change in our system.

Can I make an appointment via the Patient Portal?

At this time, we do not offer this feature. If you would like to make an appointment either as a new or as an existing patient, please call our office at 813.251.8444.

I had blood work done last week but the results are not yet posted on my Medical Summary. When will these results be available?

Some results take longer than others to come back from the lab. Once results are sent to our office and your physician has reviewed them, they will be available on the Patient Portal.

How do I request a medication prescription refill?

Under My Health, select Medications. There, you can select the Refill Medications button. Note that the Refill Medications button will only show up if your practice takes online refill requests at this time. If this button is not there, you will need to call the office directly.

How do I ask a question of my physician or the office?

Under My Messages, select New, and from the I would like to… list, choose the appropriate question.

Where do I see my lab results?

Lab results will display under My Health, in the Summary page, under the Tests and Results section. If you do not see your lab results, contact your care provider.

Two or more of my care providers use MyHealthRecord.com. Do I have to make a different login for each?

No. You can access both sets of records using one login to MyHealthRecord.com, and switch between them using the Welcome or Viewing menu in the Navigation bar (“Welcome, Jason” or “Viewing Jessica’s Profile”).

Why can't I see attachments on my messages?

Make sure that cookies are enabled on your browser.

How do I pay my bill?

If your provider accepts online payments, you will see a Pay My Bill option in the Navigation bar.

How do I request my medical records?

Under My Messages, select New and from the I would like to… list, choose ‘request a medical record’.

How can I see my medical summary? How do I send this to another medical practice?

You will see your medical Summary as soon as you log in to MyHealthRecord.com.

To send your medical information, use the Send Health Record button. Be sure to read the on-page instructions carefully. If you don’t know the email address, you will need to ask the physician you are trying to send your records to.

To download your medical record, use the Blue Button. You can save information to an SD card or a thumb drive, and give it to the other practice by hand. You can download data as a PDF and as an XML file.

How can I see the records for a specific visit? How do I send this to another medical practice?

After logging into MyHealthRecord.com, select Visit from the menu and pick a visit from the list.

To send this visit’s information, use the Send Health Record button. Be sure to read the on-page instructions carefully. If you don’t know the email address, you will need to ask the physician you are trying to send your records to.

How do I download my health record?

Under My Health, in the Summary page, select the Blue Button to download your health record as shown. You can choose to download it as a PDF or XML file. Choose PDF to make it easy to print. Choose XML to make it easy for the physician’s office to import your record into their system.
